Giant right coronary artery aneurysm with vena cava superior fistula: a case report and radiological findings

Abstract Background Giant coronary artery aneurysms (GCAA) are a rare disease entity with an estimated incidence of 0.02%. Atherosclerosis is the most common underlying factor in adulthood. Management guidelines lack support large-scale studies. Case presentation We present case 58-year-old Caucasian male complaints stable dyspnea who was found to have GCAA right (RCA). Further evaluation revealed aneurysm 5.0 cm diameter tortuous course, fistulation distal vena cava superior, and mass effect on left atrium. Surgical deroofing ligation venous bypassing were performed. There no postoperative complications. Cardiac function had improved at 1-month follow-up remained 1-year follow-up. Conclusions Diagnosis treatment strategy concerning remain challenging. advised cases giant aneurysms, multivessel disease, main (LMCA) involvement, mechanical complications (fistula, compression, or rupture), concomitant valve surgery. Coronary angiography remains gold standard for evaluation. However, computed tomography (CCTA) cardiac magnetic resonance imaging (CMR) can add important value clinician assess myocardial viability planning surgical intervention.
